Signs of Timing, Concentration and Motor Processing Deficits Checklist

Print this page, then check the signs that apply to your child, placing a (+) in the box if symptom occured in the past,
an (x) if the symptom is currently happening.

Also, please fill out the Questionnaire.

Please bring all these printed documents with you to your consultation.

This checklist should not be used in determining if a child has a Timing, Concentration and Motor Processing Deficit if the child has previously diagnosed as having Mental Retardation or Pervasive Developmental Disorder.

Check all the signs that apply to your child:
Clumsiness
Uncoordinated
Delayed mastery of sitting, crawling and/or walking
Delayed mastery of tying shoes, buttoning shirt and/or using utensils
Delayed response time for verbal and visual motor tasks
Problems staying in rhythm when singing or dancing
Problems staying in rhythm while tapping and clapping
Snappy, chaotic motor movements
Previously diagnosed as having Attention Deficit Disorder, with or without Hyperactivity
